Care Resources Program of All-inclusive Care for the Elderly (PACE)
The Dominican Sisters ~ Grand Rapids saw the need for a PACE program in Kent County. They became partners with four community organizations to found Care Resources and bring PACE to Kent County: HHS Health Options, Holland Home,Metro Health and Pine Rest Christian Mental Health Services. Care Resources opened its doors in 2006 with the help of the Dominican Sisters ~ Grand Rapids and their above-named partners.
To receive services through Care Resources, a person must be:
- Fifty-five years of age or older
- A Kent County resident
- Able to live safely at home
- Eligible for nursing-facility level of care
The care is very high quality, hands-on and holistic. Once enrolled in the program, an Interdisciplinary Team manages and oversees the participant’s health care. If a person is Medicaid-eligible, there are no out-of-pockets costs as long as the person receives care within the Care Resources network of providers.
